Physician Certification Process 

1. Schedule a Evaluation Appointment with your primary care physician or other qualified healthcare provider.

Medical Program Registry Process 

1. Wait for your approval email: Once you have your certification, you will receive a confirmation via email to register in the Patient Portal registry;

3. Complete enrollment process.  

4. Save your Registry Verification ID to your device.

Complete your Self-Evaluation in your patient portal once per quarter. This is required for all new patients for the first year.
